Canada’s Island Health is trying a novel approach to using remote patient monitoring technology to offer Hospital at Home services to more patients. The program aspires to reduce costs and improve outcomes by allowing patients to receive acute care at home instead of in the hospital. The program takes a different approach than others in the US and Canada, many of which look to identify specific patients or populations to monitor, assign the appropriate devices and establish care benchmarks. At Island Health, the focus is on creating a home monitoring routine for the patient, with devices and data points specific to that patient.
